If you need receipts before you make your request, you can use this Tier 3/5 blank HSIP receipt.  

In order to provide evidence of distribution, receipts are created upon making your request for funds in the HSIP system. The information required on the receipt is determined by the study tier level. Tier levels are explained in the HSIP request form and are assigned to a study by the IRB based on payment amount and study risk. The HSIP system will produce receipts when you make your request
