Effort Reporting Procedures - Alternate Signer Process

To seek an exception for employees not available to certify their own effort and have someone else certify on their behalf, please request an alternate signer by emailing effort.reporting.payroll@umich.edu with the following information:

Employee Name:

Administrative Dept ID: 
Certification Period: 
Termination date or date employee went on Long Term Disability (LTD):
Reason the unit cannot contact employee by fax, mail, or any form of written communication:

Proposed Alternate Name:
Empl ID: 
Brief explanation how the alternate had suitable means of verifying that the employee performed their work during the certification period.  (Simply stating that the alternate had suitable means is not sufficient.):

When the request is approved, you will receive an email notification granting permission to the alternate signer.
